Your current contact lens prescription

A contact lens prescription is typically valid for 12 months from the date it is written.

Your eye doctor's name and telephone number

We call your doctor to verify your prescription.
For fastest processing, you can take a pic of your prescription and email it to RX@contactsdirect.com.

Your insurance member I.D. number

Don't know your insurance member I.D. number?
We can look it up for you using the name, date of birth, and last four numbers of the social security number of the policy holder.
